I authorize Mamas for Mamas to release my personal information including my first and last name, e-mail address and phone number to a Direct Mamas for Mamas Holiday Client that we will be directly supporting for the Holiday season of 2023.

This information may be used only for the purpose of facilitating a pickup, drop off the Holiday Hamper.

This consent is not automatically renewable. It expires automatically at the end of the Holiday period specified unless revoked in writing sooner. By signing below, I affirm that I have read this release, or it has been read to me, and I understand its content.
